UPDATE: In order to minimize risk and person to person contact all outpatient services (evaluations, individual and group therapy) will be delivered via telephone or video conference.

To access outpatient services individuals may call 315-342-4489 or 315-342-2370 and you will be connected to the next available clinician for an assessment.

Access to medication assisted treatment will be discussed and arranged during the assessment process. Peer supports are available over the phone.

The Opioid Treatment Program (OTP) remains open for dosing and take home privileges are being increased where safe to do so. Individual and group therapy will be delivered via telephone or video conference.

To access OTP services individuals should show up at 283 West 2nd St. in Oswego on Monday at 8:30 AM or Tuesday at 11:30 AM or call 315-342-4489 for more information.

Prevention services is developing educational video content that will be rolled out via social media. Prevention counseling is also available over the phone. Please call 315-342-4489 to access Prevention Services or for more information.

OPEN ACCESS - What is it?  And how does it work?

"Walk in" to either Farnham location for an immediate connection to services during Open Access hours. During Open Access you will complete an intake, receive a comprehensive evaluation from a qualified health professional, and be connected to a certified peer advocate.

Hours - Monday, Tuesday, Thursday Noon - 3 p.m.

Support - Tell us your story and our caring staff will help determine which level of care works best for you and get the right level of support to begin your recovery journey. Our peer advocates are available to provide support from your first visit to Farnham.

Insurance - Don't have insurance? No worries. We will assist you in obtaining insurance. No one is refused services because of inability to pay. If you have insurance, bring your insurance information.

Other Services - If you need to access services outside of the Open Access hours please contact us. If you are interested in the Opiate Treatment Program please call us at 315-342-4489 to schedule an initial assessment.
